1) Notice of Financial Incentive

1.1 Summary of the incentive

The Program is a voluntary rewards offering. Eligible participants can earn points and redeem them for rewards (primarily physical items, and sometimes digital codes and discounts).

1.2 Material terms of the Program

You can earn points for eligible actions described in the Rewards Store, including verified purchase activity.

Points are non-transferable, not redeemable for cash, and have no cash value.

Rewards availability and redemption options may change over time.

1.3 Categories of personal information involved

Depending on how you use the Program, we may collect/use:

Identifiers (e.g., name and email address for account registration)

Program data (points balance, redemption history, and support interactions)

Purchase verification data (e.g., delivery note/order confirmation identifiers used to confirm eligibility and prevent duplicates)

Usage analytics (app usage and technical logs used to improve user experience)

1.4 Good-faith estimate of value & method used

California law requires that a financial incentive be reasonably related to the value of the consumer's data, and that we disclose a good-faith estimate and the method used to calculate that value.

Our method (good-faith): We estimate the value of consumer data in connection with this Program using a reasonable method based on:

• the average value of rewards actually redeemed per active participant over a 12-month period, minus reasonable program administration and fulfillment costs; and
• the Program's operational value in enabling eligibility verification and fraud prevention.

Because redemption behavior varies by participant, Miltenyi can provide the then-current estimate upon request by contacting orderdesk@miltenyi.com.

1.5 How to opt in

For new account registrations, you opt in voluntarily by affirmatively checking the registration consent confirming that you agree to the Rewards Store Terms & Conditions, acknowledge the Privacy Notice, and voluntarily opt in to the Rewards Store program described in this Notice of Financial Incentive. Existing members who enrolled before affirmative consent capture are asked to review the current Program documents and provide the same express opt-in before continuing Program participation. The Rewards Store records the server-side acceptance timestamp and the document/version markers associated with each affirmative consent.

1.6 Right to withdraw

You may withdraw at any time by closing your Rewards Store account or contacting orderdesk@miltenyi.com. If you withdraw, you will stop earning points and may forfeit unused points as described in the Terms.
